Prescription refills online in Alberta: what a virtual physician can and cannot do

How prescription renewals work in a virtual visit, which medications may need an in-person review, and how to prepare so the visit goes smoothly.

Renewals are a medical decision

A prescription renewal is a clinical decision, not a form to fill. In a virtual visit the physician reviews your history, the medication, how you are doing and whether monitoring is due. When it is appropriate, the prescription can be sent to the pharmacy you choose in Alberta. When it is not, the physician explains what is needed first, such as blood work, a blood-pressure check or an in-person examination.

Medications that may not be renewed virtually

Some medications carry extra safeguards. Controlled substances such as opioids, benzodiazepines and stimulants are generally not started, and often not renewed, through a first virtual visit. Medications that require regular physical examinations or lab monitoring may need an in-person review before renewal. The physician follows the College of Physicians & Surgeons of Alberta standards on prescribing and virtual care.

How to prepare

Have the exact names, strengths and doses of your medications, or the pharmacy labels, in front of you. Know the name of your pharmacy. Mention any side effects, missed doses or changes in your health since the last prescription. If your family physician manages the medication, say so: continuity of care matters and the clinic may recommend that you follow up with them.

Coverage and fees

A medically necessary visit to review a medication is an insured service for eligible Alberta residents. Patients without valid Alberta Health coverage may be charged the private consultation fee of $75 CAD. The pharmacy charges for the medication itself and any dispensing fee according to your drug plan. If you have a severe reaction to a medication, such as trouble breathing or swelling of the face, call 911.

Can I get a prescription refill without seeing a doctor?

No. A physician must assess whether the renewal is appropriate, but the assessment can often happen in a video visit.

Can a virtual doctor send the prescription to my pharmacy?

Yes. When a prescription is appropriate, it can be sent electronically or by fax to a pharmacy in Alberta.

Will I get a new prescription if I run out?

Not automatically. Book early rather than waiting until the last dose, and ask your pharmacy about an emergency supply if you are already out.
